Cancellation Form

WORKOUT/NUTRITION CANCELLATION POLICY

By choosing to cancel your account, your membership with Lean For Life will end 30 days after your cancellation form is submitted. If your membership date* is within the 30 day cancellation period, your payment will be a pro-rated charge reflecting your membership date through your cancellation date.

There are no membership freeze/hold options.

*Membership date refers to the date you signed up with Lean For Life Lia and is directly correlated to your payment date.
